What you'll need

Baby on changing table or safe flat surface with diaper supplies within reach. Baby should be alert and calm — not hungry or overtired. Parent positioned so baby can see their face clearly.

How it works

  1. 1~30s

    Before you start changing the diaper, get your child's attention by making eye contact. Then slowly stick out your tongue — make it really obvious and hold it for a few seconds. Watch their mouth closely. Does your child try to stick out their tongue too? Even a little tongue movement or lip smacking counts! Tell me what you notice.

    Watch for: Baby attempts to imitate parent's facial expression, particularly tongue protrusion.

  2. 2~25s

    Now as you begin changing the diaper, try a big, warm smile. Look right at your child and smile with your whole face — crinkle your eyes, show your teeth. Does your child smile back? Watch for that social smile — not just a reflex, but a genuine smile in response to your expression. Tell me about their reaction.

    Watch for: Baby smiles in direct response to parent's smile, showing social reciprocity.

  3. 3~35s

    As you finish the diaper change, try a few more expressions — maybe open your eyes wide in surprise, or make a kissing sound with your lips. Notice if your child stays engaged with you during the whole process. Does they watch your face, make eye contact, seem interested in the interaction rather than just the physical care? Tell me how your child participates in this social diaper time.

    Watch for: Baby remains socially engaged with parent during routine caregiving activity.
